Our office hours are 8:00am-6:00pm Monday through Thursday, 8:00am-5:00pm on Friday, with urgent care available Saturday 9:00am-noon on Saturday.
We make every effort to respect your valuable time. As such office visits are arranged by appointments. On occasion emergencies or unexpected circumstances may cause delays beyond our control. We ask for your patience and understanding in these situations.
We request a 48 hours notice for medication refills. This allows us ample time to review your health record to ensure that your refill is accurately and appropriately completed.
Emergent/After Hours Care
One of our physicians is available 24 hours a day seven days a week to address any after hours issues as well as any urgent or emergent medical concerns requiring immediate attention. If such service is needed, please call the office at 884-2133. The on call physician will be paged and will promptly return your call.
Fees & Insurance
Payment of the fees associated with your visit is expected at the time of service. Please bring all applicable insurance information, your social security number and drivers license for proper identification.
We will gladly file any insurance that can be processed electronically. To accurately submit your claim, we require documentation of valid and current insurance coverage and personal identification in the form of a drivers license and social security number. Otherwise, we will supply you with a printed receipt for services that you may file with your insurance independently. To assure current information we also require annual update of insurance coverage at the first visit of the year.
We fully realize that hardship can fall to anyone. Should you find yourself in financial difficulty please contact our billing office. We will assist you in designing a reasonable monthly payment plan.
